Upshur Emergency Preparedness Registry

 

DHSEM-logo-200x200 Thank you for your interest in the Emergency Preparedness Registry.  This program is designed for individuals with special medical needs and who may require specialized assistance in the event of an emergency and/or a disaster. 


The Upshur County DHSEM maintains a list of individuals who may need assistance during an emergency situation. This information is only shared with other emergency service agencies (local fire departments, law enforcement, emergency medical services, 911) as needed. This information allows the agencies to better prepare to respond to the community.

All individuals should anticipate the need for evacuation and make advanced arrangements to shelter with relatives, friends and/or community organizations. The specialized assistance provided by emergency service personnel via this program is intended as a temporary measure when, for reasons beyond your control, you are unable to implement your own emergency plan. Individuals requiring a personal caregiver for specialized assistance during an emergency and/or disaster should always be accompanied by that personal caregiver. Administrators of nursing, convalescent, assisted living, retirement and other group facilities are responsible for the evacuation and sheltering of their own residents.

This program requires an annual enrollment or update. Please complete the registration form below and return it to us via mail, fax, or email.
